Most Important – Only answer what you want to answer.
Why Limit? – These forms often have very little to do with your specific treatment. The data is often used for research, profiling, government reporting and predictive analytics without your consent.
Choose – Use your best judgment as to what information, if any, you want to share.

You can send your questionnaires to us. Unaltered forms are preferred, however, if the form is completed we will redact all personal information.
